It's like Deftones would go Slowdive and had some emo influences at the same time - they were way ahead of their times

I'd say the only Deftones-like track on there is C.O.R., which sounds like an intentional parody. But it's a very great album, the songs sound concise and punchy, and I love the hopeful mood of it. Not a single skippable track too.

does anyone else make shoegaze with the kind of harsher, sharper sound that Medicine does? not quite pure noise rock, still droney and space-filling, but also not 'soft' the way dream sounds

Skywave and APTBS use that kind of noises very often, but their songs are more punchy. Same with Screen Vinyl Image. You should try Ceremony (East Coast), but their songs are way more "poppy" and less experimental in sound than Medicine's.
And you always have JAMC and bands like The Raveonettes. Xin Li Supreme is the harsher noise of this kind.
